Above the notation, the title is given as "Herr Jesu Christ, du höchstes Gut." In the Register, there are two titles that begin this way ("Herr Jesu Christ, du höchstes Gut, du" and "Herr Jesu Christ, du höchstes Gut, von"), and I don't know which is the actual title of this tune.
As I've noted before, the different sections of alternate notes can be combined in different ways, but I always try to squeeze as many as possible into a single iteration. In the notation for this tune, I put some lines to indicate where two sections of alternate notes abut, which I really should have been doing all along.
